Our purpose;

• Provide exceptional support coordination services for its individuals and families receiving services.
• To coordinate the delivery of supports to persons receiving services from Utah DSPD, Medicaid; Intellectual Disabilities and Related Conditions(ID.RC) Waiver Services and to monitor the health and safety of those persons.
• To offer a stable and flexible working environment for Support Coordinators-External, and foster an atmosphere of open communication, mentoring, and support.
